My TLC number is 5081212 based on the application which I filed on April 27, 2001 for a TLC license. I paid $500 in fees and completed the 80 hour course and passed the drug tests, physical examination and written test with a very high score and was waiting for my license to arrive in the mail when I learned from Miss Cipulyam of the TLC that they had "found" this old ticket. Had I known about this ticket before paying the $500 and taking the 80 hour course, I could have done something about it or else abandoned my plans to become a taxi driver.
The decision states: "According to TLC records, he filed an application & was assigned TLC license # 5061483."
This is completely false. I never filed any such application. No such number was assigned to me. Not one word of this is true. I demand that you produce a copy of this application that it states that I filed. Nothing of the kind took place.
The decision states that the notice was sent by certified mail. However, the notice itself says that it was sent by regular mail. I demand that you produce the certified mail receipt, because there is none. No such thing exists.
The decision denying my motion to vacate must be reversed.
